*Position: *Program Manager of Residential CLA
*Supervised by: *Director of DDS Services
*Schedule- *Monday-Friday/9am-5pm Exempt
*Rate: *$57,000/Annual Salary
*General Description of Position:*
To manage Community Living Arrangements Home (CLA) under the guidance of The Department of Developmental Services.
This includes all staff, supervisors, nursing, and any other pertinent member to be added.
To be a facilitator, ensuring that all the individuals served are integrated, dignified, and respected in all environments and are receiving the necessary support to be active in their chosen environments.
These supports to the individuals will be provided with dignity and respect per their personal preferences in accordance with TLA’s policies/procedures and with State/Federal mandated laws, policies, and regulations.
To train, teach and keep structured systems in place for all programmatic needs.
*Essential Responsibilities:* * Strives to continually work as an active, productive team member to ensure success of the individuals and the agency utilizing enthusiasm, creativity, and professionalism.
* Follows and demonstrates the mission of TLA.
* Adheres to TLA and DDS Policies and Procedures.
* Promotes choice, provides appropriate support, and coordinates activities to foster individual wellbeing, growth, and development.
* Represents the agency positively and professionally when interacting with individuals served such as TLA staff.
This would also include families, guardians, DDS and outside vendors.
* Communicates effectively, clearly, and positively with individuals served, families, staff, supervisors, and community members.
* Participates in all licensing and DDS Quality Service Reviews.
* Responsible for program quality service reviews which includes all safety documentation, environmental safety oversight and staff training records are up to date.
* Manages the petty cash and purchases for the programs, keeping within the guidelines and budget.
* Manages the personal finances for those individuals residing in the CLA home.
* Ensures the protection of all individuals rights and safety by following HIPPA regulations, ensuring privacy and reporting any type of abuse or neglect.
* Attends, completes, and demonstrates competency in all applicable TLA training.
* Advocates for the individuals whenever necessary as well as teaching the individuals to advocate for themselves.
* Oversees the supervision of the individuals’ daily living skills including but not limited to personal care, recreation, meal prep, and household chores.
* Ensures that the individual's plan is carried out as outlined in the written plan, programs, schedules, and other documentation.
* Schedule interviews for new hire opportunities.
* Plan, coordinate and conduct new hire training.
* Ensure that all staff are trained, and shadow shift are completed prior to working with individuals served alone.
* Attend all resource and DDS Meetings per regulatory needs.
* Complete monthly billing as assigned.
* Must be fluent in Therap case management system which includes auditing and maintaining all documentation to TLA standards.
* Record and maintain individuals’ records in Therap as assigned such as data entry, case notes, updated contacts, and information.
* Administer medications in accordance with DDS regulations.
Ensuring all medications are available and orders followed.
* Intervene in emergency or behavioral situations as needed.
* Ensure that GERs, DDS 255, and critical incident reports are completed in a timely manner.
Ensures GER solutions are completed in a timely manner.
* To be able to manage staffing schedules including call outs.
* To be able to be in ratio if needed.
* To be able maintain staffing: Individual ratios per each person’s LON.
* Responsible for all State and agency documentation to be completed accurately and submitted in a timely manner.
* Responsible for 90 day and annual evaluations for all staff.
To be submitted in a timely manner based on an employee’s hire date.
* Must be comfortable with giving disciplinary actions to insubordinate staff.
*Other Related Responsibilities:* * On-call rotation * Attend all applicable agency meetings.
* Excellent written and verbal communication skills.
* Demonstrate appropriate time management skills.
* Maintain professional demeanor and act as a role model for employees.
* Ability to respond appropriately to emergency situations and day-to-day management and supervisory issues.
* Keep informed and implement current trends and methods of working with individuals with disabilities.
* Have a good relationship with others- work as a team player.
* To be able to work in a hostile environment.
* To be able to lead the program staff to perform task to the best of their abilities.
* Maintain a positive attitude.
*Minimum Education Preferred: *A Bachelor’s degree preferred in Human Services or related field and/or 5 plus years in Human Services working with Individuals with Developmental Disabilities.
*Required Knowledge Skills and Abilities* * Must have a valid State of Connecticut Driver’s License and be insurable with the agency carrier.
* Must be able to meet physical demands of the job.
These include, but are not limited to: the ability to lift minimum of 50 pounds for assisting participants in personal care needs and/or the purpose of performing daily job duties, the ability to twist, bend and reach immediately to address medical and behavioral incidents, ability to enter and exit agency vehicle at numerous stops for a minimum of one hour and the ability to drive agency vehicle for a minimum of 30 minutes without stops.
* Ability to follow oral and written instructions.
*Additional Requirements* * Must complete the Agency’s Employee Training.
* Certification and Training, including but not limited to CPR, First Aid, PMT and Med Cert.
